Thanks. The ./sbin/rabbitmq-plugins list output
above suggests that the plugin is enabled but seems to fail load
dependencies under $RABBITMQ_BASE/plugins.

Please try using an RPM or getting a generic UNIX tarball and trying that with your
custom settings:
