This indicates a serious inconsistency in RabbitMQ's database.

Would you mind posting a zip/tgz of the database dir 
(/usr/local/var/lib/rabbitmq/mnesia/rabbit at localhost) somewhere?

After that, if you want to get going and don't mind losing existing 
data, simply remove that dir.
