There are two problems...

1) Your version of Erlang is too old. As shown by ...

... which is R12B-5. You need at least R13B01 in order to run the 
management plug-in. See http://www.rabbitmq.com/which-erlang.html.

2) You did not install the 'mochiweb' plug-in, only 'rabbitmq_mochiweb'. 
The latter depends on the former.
